In accordance with state and federal privacy laws it also includes all protected health information of member name, address, date of birth, health plan member number, social security number, medical record number, phone or fax numbers, dates of service, any uniquely identifying number, characteristic or code and any other data and information contained in, disclosed by, or displayed through the Children's Health Network (CHN) Referral System.
